TRAUMA CARE INITIATIVE

BACKGROUND AND NEED/NECESSITY:

  • Trauma is one of the leading causes of death and disability in world with more than 1.5 million deaths and almost 20 million serious injuries/ year just from the road traffic accidents and violence.
  • Mortality and morbidity rates are 5-6 times higher for seriously injured in the under developed countries like Pakistan compared to USA.

  • In Pakistan pre-hospital trauma care is lacking and first responders lack training
Even in hospitals, majority of doctors, nurses and other health care individuals working in emergency departments are not appropriately trained in trauma care and lack ATLS, PTC, BCLS or ACLS certification

HOW CAN WE IMPROVE?:

It is possible to avoid most of trauma related morbidity, disabilities and deaths by low cost measures such as better training of first responders and hospital personals and better planning for trauma care. It can be achieved by development of a sustainable, affordable and effective pre-hospital trauma care system and by improvement and training of hospital based system/personals.

GOALS OF THE PROJECT:

  1. Cost effective, local resources, basic training to first responders (ambulance teams, police, volunteers, taxi drivers, teachers, paramedics etc.) to provide the initial help on the scene by individuals trained in basic principles of safe rescue, first aid and transportation.
  1. To establish minimum standards of training, certification required equipment and supplies to first responders with at least BCLS (Basic Cardiac Life Support) but preferably PTC (Phenylthiocarbamide).
  1. Pre-hospital trauma care will need to be supported by well established hospital based trauma care system which should have well trained and certified staff in emergency departments who are capable of managing trauma victims. Goal is to have the qualified doctors and nurses to undergo well defined trauma care training such as ATLS or PTC along with basic cardiac and life support (BCLS) training. Such certification should be part of residency programs for all students and trainees working in ED.
  1. Trauma care protocols should be established and applied in all participating hospitals to achieve successful results.
  1. Trauma Registry to be established to create reliable data for outcome analysis and to implement improvements in the system.

HOW TO ACHIEVE GOALS:

  • Will identify a primary person in each participating hospital and city for oversight.
  • Will train a group of certified trauma trainers in each city with PTC certification. Total of 20 Trainers/ City will be our sustainable core group.
  • Those trained and certified core groups will take a lead in educating and training first responders and will also train nurses and physicians especially those working in emergency departments in each city on trauma care with the goal of about 1000 people trained every year/city. Training will include BCLS or PTC certification for first responders and PTC certification for health care professionals (physicians, student, trainees and nurses) and ATLS certification for faculty and team in charge (registrars in emergency medicine and surgery etc.). Recertification will be every 3 year.
  •  Will start with hospitals in major cities including Islamabad/ Rawalpindi Peshawar, Lahore, Faisalabad, and Karachi but will expend initiative to throughout the country with the help of NGO, medical societies and alumni. Will expend the program as resources are developed to budget the program expansion.
  • Al-Masoom will be contacted and asked if their medical schools /hospitals like to participate in this project and if they like to share the cost of this training/certification at their institutions.
